Diferentes métodos para evaluar el aumento de peso gestacional y su asociación con el peso al nacer

Resumo

Objetivo

Analisar a associação de diferentes métodos para avaliação do ganho de peso gestacional com nascidos vivos pequenos para idade gestacional (PIG) ou grandes para idade gestacional (GIG).

Métodos

Estudo transversal, com mulheres adultas, IMC pré-gestacional de eutrofia, gestação única e idade gestacional no parto ≥28 semanas, da pesquisa ‘Nascer no Brasil’, em 2011-2012.

Resultados

Participaram do estudo 11 mil mulheres; a prevalência de ganho excessivo foi de 33,1% segundo os métodos Brandão et al., e IOM, e 37,9% segundo Intergrowth. A chance de nascer PIG para ganho de peso insuficiente foi de OR=1,52 (IC95% 1,06;2,19), OR=1,52 (IC95% 1,05;2,20) e OR=1,56 (IC95% 1,06;2,30) para Brandão et al., IOM e Intergrowth, respectivamente, enquanto o ganho de peso excessivo apresentou OR=1,53 (IC95% 1,28;1,82), OR=1,57 (IC95% 1,31;1,87) e OR=1,65 (IC95% 1,40;1,96), para GIG, respectivamente.

Conclusão

Comparados às recomendações do IOM, Intergrowth e Brandão et al. apresentam-se como alternativas para identificar PIG e GIG.

Resumen

Objetivo

Analizar diferentes métodos de evaluación del aumento de peso gestacional (APG) con nacidos vivos pequeños para la edad gestacional (PEG) y grandes para la edad gestacional (GEG).

Métodos

Estudio transversal, con mujeres adultas, IMC pregestacional eutrófico, un solo embarazo y edad gestacional al nacer ≥28 semanas, del estudio `Nacer en Brasil´, entre 2011 y 2012.

Resultados

En las 11.000 mujeres del estudio, la prevalencia de ganancia excesiva fue del 33,1% según los métodos de Brandão et.al. y el IOM y 37,9% para Intergrowth. La probabilidad de nacer PEG por una ganancia de peso insuficiente fue OR=1,52 (IC95% 1,06; 2.19), OR=1,52 (IC95% 1,05; 2.20) y OR=1,56 (IC95% 1,06; 2.30) para Brandão et.al, IOM e Intergrowth. La ganancia de peso excesiva, en los mismos métodos presentó OR=1,53 (IC95% 1,28;1,82), OR=1,57 (IC95% 1,31;1.87) y OR=1,65 (IC95% 1,40;1,96) para GEG.

Conclusión

En comparación con las recomendaciones del IOM, Intergrowth y Brandão et.al. se presentan como alternativas en la identificación de PEG y GEG.

O ganho de peso gestacional (GPG) reflete múltiplas mudanças decorrentes da gestação, entre as quais se incluem o acúmulo de gordura corporal na mulher, a expansão de líquidos, o desenvolvimento fetal e placentário, o aumento do tecido mamário e do útero gravídico.11. Os recém-nascidos classificados como PIG podem apresentar prejuízo no desenvolvimento neurológico, performance escolar abaixo do esperado, baixa estatura resultante de deficiência de hormônio do crescimento e, na vida adulta, maior chance de síndrome metabólica.55. O objetivo deste trabalho foi analisar a associação de diferentes métodos para avaliação do ganho de peso gestacional com nascidos vivos pequenos para idade gestacional (PIG) ou grandes para idade gestacional (GIG) no Brasil.

Métodos

Trata-se de um estudo transversal, desenvolvido com dados da pesquisa ‘Nascer no Brasil’, realizado com puérperas no período de fevereiro de 2011 a outubro de 2012. Foi selecionada uma amostra complexa, que envolveu 266 hospitais e 90 mulheres em cada uma dessas unidades, totalizando 23.940 entrevistadas. Esta amostra foi ponderada pelo inverso da probabilidade de inclusão de cada puérpera, e calibrada de forma à distribuição das puérperas incluídas ser similar à observada no ano de 2011. A população do presente estudo foi constituída por mulheres adultas (≥20 anos), com gestação de feto único, nascido vivo, idade gestacional ao nascimento a partir de 28 semanas e IMC pré-gestacional de eutrofia (18,5 a 24,9kg/m2). O IMC pré-gestacional (kg/m2) foi calculado com base no peso pré-gestacional, dividido pela estatura elevada ao quadrado.11. World Health Organization - WHO. Foram excluídas as mulheres que não dispunham de informações do peso ao final da gestação, medida indispensável ao cálculo do ganho de peso gestacional total.

Para compor a variável ‘peso pré-gestacional’ (kg), considerou-se o valor descrito no cartão e/ou caderneta da gestante; caso o peso pré-gestacional fosse desconhecido, adotou-se o peso medido até a 13ª semana gestacional ou o peso referido pela mulher durante a entrevista.1010. Para a estatura, considerou-se o dado registrado no cartão ou o referido pela mulher no momento da entrevista.

As medidas referidas foram validadas em estudo anterior, realizado com a mesma população. Naquela ocasião, foi constatada boa correlação entre as informações do cartão (padrão ouro) e as referidas pela puérpera: um coeficiente de correlação intraclasse (ICC) de 0,96 para peso pré-gestacional; e de 0,89 para estatura.1414. Para o tratamento dos dados faltantes do peso pré-gestacional e da estatura, empregou-se o método de imputação de dados. Na amostra inicial (n=23.940), 17,5% das mulheres tiveram um dos dados imputados (15,5% de dados da estatura materna e 4,4% de peso pré-gestacional), pelo método de chained equations (MICE).1515. As variáveis usadas como potenciais confundidoras na relação entre o ganho de peso gestacional e os desfechos PIG e GIG foram: macrorregião do país (Norte; Nordeste; Sudeste; Sul; Centro-Oeste); fonte de pagamento do parto (pública; privada); idade materna (em anos: 20 a 24; 25 a 29; 30 a 34; 35 a 39; 40 ou mais); raça/cor da pele (branca; preta; parda; amarela; indígena); escolaridade (em anos de estudo: até 7; 8 a 11; 12 a 14; 15 ou mais); número de consultas de pré-natal (não fez pré-natal; 1 a 3; 4 a 6; 7 ou mais); idade gestacional (em semanas: 28 a 31; 32 a 36; 37 a 41; 42 ou mais); tipo de parto (normal; cesáreo); paridade (nuliparidade; 1 a 2 partos prévios; 3 ou mais partos prévios); tabagismo durante a gestação (sim; não).

A média do ganho de peso gestacional foi calculada pela análise de regressão logística univariada para amostras complexas, utilizando-se o teste de Bonferroni para avaliar a significância estatística. As diferenças entre as prevalências dos desfechos de PIG e GIG foram avaliadas pelos intervalos de confiança de 95% (IC95%) e pelo teste do qui-quadrado de Pearson. Adotou-se nível de significância de 5%. O peso e o desenho amostral foram considerados em todas as análises.

Foi realizada análise de regressão logística para cada um dos desfechos - PIG e GIG -, tendo como principal variável de exposição o GPG; estimou-se a razão de chances (odds ratio [OR]) bruta e ajustada, com IC95% para todos os desfechos, utilizando-se como referência todas as participantes que apresentaram adequação de GPG (P30 a P70). Os modelos foram ajustados pelos potenciais confundidores. Apenas os modelos das recomendações do IOM2 e da proposta de Brandão et al.1010. O estudo fundamentou-se em dados da pesquisa ‘Nascer no Brasil’, cuja amostra foi submetida ao Comitê de Ética em Pesquisa da Escola Nacional de Saúde Pública Sergio Arouca, da Fundação Instituto Oswaldo Cruz (ENSP/Fiocruz), e aprovada pelo: Protocolo do Certificado de Apresentação de Apreciação Ética (CAAE) nº 92182418.9.0000.5240, emitido em 8 de julho de 2018. A pesquisa ‘Nascer no Brasil’ apresentou Termo de Consentimento Livre e Esclarecido para assinatura da mulher, antes da realização da entrevista.

Resultados

Entre as 23.940 mulheres, 11.034 atenderam aos critérios de inclusão da amostra; 34 delas não apresentavam a informação do peso gestacional final, indispensável para o cálculo do ganho de peso gestacional total, não sendo possível a imputação dessa variável para estas mulheres. Por este motivo, a amostra final do estudo contou com exatas 11 mil participantes, cujas características estão descritas na Tabela 1. Observa-se que 42,7% das mulheres consultadas residiam na região Sudeste, 75,9% tiveram parto na rede pública de saúde, 38,0% apresentavam idade entre 20 e 24 anos, 55,1% eram de raça/cor da pele parda, 45,1% tinham de 12 a 14 anos de estudo, 60,8% receberam 7 ou mais consultas de pré-natal e 53,2% submeteram-se a parto cesáreo.

Pelos três métodos, as mulheres com ganho de peso insuficiente referiram maior prevalência de crianças pequenas para a idade gestacional (PIG), comparadas àquelas que ganharam peso adequado. O ganho de peso gestacional excessivo, contraposto ao ganho adequado, evidenciou maior prevalência de GIG: 18,5% contra 12,2% para Brandão et al.;1010. está em propiciar o acompanhamento do ganho de peso gestacional ao longo da gravidez e não considerar o ganho ponderal como sendo constante, em todo o período. Contudo, é necessária a extensão da proposta para as demais categorias de IMC pré-gestacional, assim como estudos de validação que estimem o desempenho do GPG na predição da adequação do peso ao nascer em diferentes populações.

A maioria dos grandes estudos epidemiológicos de avaliação do ganho de peso gestacional dispõem do peso pré-gestacional e do peso ao final da gestação, sendo possível apenas o cálculo do ganho gestacional total. O tema ganho de peso gestacional continua a ser relevante, dada a dinâmica da mudança ponderal, sendo um fator crítico e ao mesmo tempo passível de modificações nos resultados obstétricos. Outrossim, até o momento desta publicação, não são conhecidos trabalhos que tenham se utilizado dos três métodos apresentados, para os desfechos de PIG e GIG.

Comparados aos resultados da aplicação do método IOM, os ganhos de peso gestacional insuficiente e excessivo encontrados pelos padrões do Intergrowth99. e os padrões do Intergrowth apresentam-se como alternativa, para identificação de mulheres com maior chance dos desfechos de PIG e GIG, em lugar do IOM, podendo auxiliar na qualidade da atenção pré-natal em mulheres brasileiras com características semelhantes às deste estudo. No entanto, estudos prospectivos ainda são necessários para avaliar outros desfechos em saúde.
