A crise econômica que afeta a Europa neste século XXI tem modificado os sistemas de proteção social dos países que adotaram, no século XX, o modelo de sistema de saúde universalista, como a Espanha. Esta comunicação apresenta algumas transformações recentes, causadas por mudanças na legislação espanhola. Estas se deram no acesso às prestações de saúde, principalmente com relação ao atendimento a estrangeiros, no aporte financeiro dos usuários às prestações sanitárias e na assistência farmacêutica. Verifica-se que em situações de crise há tendências de reforma no sentido de restringir direitos, aprofundando desigualdades sociais.Abstract in English:
The economic crisis that has been affecting Europe in the 21st century has modified social protection systems in the countries that adopted, in the 20th century, universal health care system models, such as Spain. This communication presents some recent transformations, which were caused by changes in Spanish law. Those changes relate to the access to health care services, mainly in regards to the provision of care to foreigners, to financial contribution from users for health care services, and to pharmaceutical assistance. In crisis situations, reforms are observed to follow a trend which restricts rights and deepens social inequalities.Abstract in English:
The Brazilian National Regulatory Agency for Private Health Insurance and Plans has recently published a technical note defining the criteria for the coverage of genetic testing to diagnose hereditary cancer. In this study we show the case of a patient with a breast lesion and an extensive history of cancer referred to a private service of genetic counseling. The patient met both criteria for hereditary breast and colorectal cancer syndrome screening. Her private insurance denied coverage for genetic testing because she lacks current or previous cancer diagnosis. After she appealed by lawsuit, the court was favorable and the test was performed using next-generation sequencing. A deletion of MLH1 exon 8 was found. We highlight the importance to offer genetic testing using multigene analysis for noncancer patients.Abstract in Portuguese:
O objetivo deste estudo foi testar se ganho de peso na vida adulta está associado à ocorrência de miomas uterinos. Foram avaliadas 1.560 participantes do Estudo Pró-Saúde. O ganho de peso foi avaliado de forma contínua e em quintis. Razões de chances e intervalos de 95% de confiança foram estimados por modelos de regressão logística, com ajustes para escolaridade, cor/raça, índice de massa corporal aos 20 anos, idade da menarca, paridade, uso de contraceptivo oral, tabagismo, plano de saúde e exame Papanicolaou. Não foram observadas diferenças relevantes na ocorrência de miomas uterinos entre quintis de ganho de peso nessa população de estudo.Abstract in English:
This study intended to investigate whether body weight gain during adulthood is associated with uterine myomas. 1,560 subjects were evaluated in a Pró-Saúde Study. Weight gain was evaluated in a continuous fashion and also in quintiles. Odds ratios and 95% confidence intervals were estimated through logistic regression models that were adjusted for education levels, color/race, body mass indices at age 20, age of menarche, parity, use of oral contraceptive methods, smoking, health insurance, and the Papanicolaou tests. No relevant differences were observed regarding the presence of uterine myomas among weight gain quintiles in that studied population.
