  • O luto antecipado diante da consciência da finitude: a vida entre os medos de não dar conta, de dar trabalho e de morrer Artigo

    Em suas práxis, os profissionais de saúde são forçados a lidar com os lutos do paciente, da família e da equipe, mas para uma pessoa idosa, o luto antecipado devido a doenças e a consciência da finitude podem ser tão perturbadores quanto a morte efetiva de alguém. O presente trabalho objetiva compreender o luto antecipado, percebido na interação entre a velhice e os processos saúde-doença e incapacidade, na visão de idosos da comunidade diante da própria finitude. Este estudo etnográfico observacional, realizado com 57 idosos assistidos pela Estratégia Saúde da Família, teve a coleta e a análise de dados guiadas pelo modelo de signos, significados e ações, em entrevistas semiestruturadas, emergindo como categorias finais: experiências da morte em vida; experiências dos idosos na atenção à saúde; consciência da finitude e lutos antecipados que tiram o sentido da vida. Para a cultura local a velhice é uma doença; incapacidade é "não dar conta" de fazer atividades cotidianas e "dar trabalho" aos outros é pior do que morrer. Quanto mais velhos, mais lutos e perdas - antecipados e reais; maior a consciência da própria finitude; realidade que tem sido negligenciada na atenção à saúde. Compreender essa questão é crucial para um cuidado humanizado e integral da pessoa idosa e das famílias.

    In their praxis, health professionals must deal with the grieving of patient, family and staff, but for the elderly anticipated grieving due to disease and the awareness of finitude can be as disturbing as the actual death of someone. This paper seeks to understand anticipated grieving in the interaction between old age and health-disease and disability processes from the viewpoint of the elderly in the community faced with their own finitude. This observational ethnographic study was conducted with 57 elderly people assisted by the Family Health Strategy. The collection and analysis of data in semi-structured interviews was based on the model of signs, meanings and actions. The final categories that were revealed were: experiences of death while alive; experiences of the elderly in health care; awareness of finitude and anticipated grieving that negate the meaning of life. In local culture aging is a disease, disability is "being unable to cope" with everyday activities, and "giving too much trouble" to others is worse than dying. The older they are, the greater the anticipated and real grieving and loss and awareness of finitude, which have been neglected in health care. Understanding this question is crucial for comprehensive and humane care of the elderly and their families.
  • Influência da idade na percepção de finitude e qualidade de vida Artigo

    Estudo de corte transversal que analisou a influência da idade na percepção de finitude e qualidade de vida (QV) de funcionários públicos, com uma amostra não probabilística por acessibilidade composta de 471 funcionários com média de idade de 40 anos. Utilizou-se um questionário autoaplicável. Os resultados da atividade física mostram que o grupo C obteve a maior média em todos os componentes e o grupo B os menores valores com exceção da caminhada. Todos os participantes do estudo apresentaram bons escores nos quatro domínios da QV (58% - 74%). Na percepção de finitude, o grupo B obteve os melhores escores (percepção positiva) nos quatro domínios, enquanto os grupos A e C apresentaram uma percepção mais negativa, não havendo diferença significativa entre as variáveis. Evidencia-se a relação positiva da finitude na faixa etária dos 35 aos 50 anos. Foi identificado QV positiva e alto nível de atividade física.

    This cross-sectional study analyzed the influence of age on the perception of finitude and quality of life (QOL) of government employees, with a non-probabilistic accessibility sample composed of 471 employees with an average age of 40. A self-administered questionnaire was used for the study. The results show that physical activity of group C had the highest average in all components and group B the lowest values, with the exception of walking. All the participants achieved good scores in the four domains of QOL (58% - 74%). In the perception of finitude, group B obtained better scores (positive perception) in the four areas, whereas groups A and C had a more negative perception, there being no significant difference between the variables. This study reveals the positive relation of finitude in the age group from 35 to 50. Positive QOL and a high level of physical activity were identified.
  • Adoecimento e finitude: considerações sobre a abordagem interdisciplinar no Centro de Tratamento Intensivo oncológico Artigo

    O adoecimento por câncer, que envolve sobremaneira tratamento invasivo e mutilador, traz repercussões na vida dos sujeitos, remontando a um imaginário de associação à dor, limitações e receio da morte, o que demanda a participação do familiar ou responsável pelo paciente durante o tratamento. O atendimento aos familiares ou responsáveis pelo paciente é um importante conteúdo da avaliação do cuidado nas instituições de saúde, enquanto aspecto integrante do processo de humanização da assistência e que requer dos profissionais a capacitação para identificar e atender suas necessidades. Diante do exposto, o presente estudo tem como objetivo trazer à tona algumas considerações sobre o trabalho interdisciplinar no acolhimento à família dos pacientes do Centro de Tratamento Intensivo adulto do Hospital do Câncer I, enquanto proposta eficiente no que tange ao atendimento de tais necessidades de informação clínica, orientação e suporte social, clínico e/ou psicológico.

    Being afflicted with cancer involves a high degree of invasive and mutilating treatment, which has repercussions on the lives of individuals and harks back to the recollection of pain, limitations and fear of death, which requires the participation of the family or person responsible for the patient during treatment. Attention to the relatives or persons responsible for the patient is a very important evaluation issue of care in health institutions, as an integral aspect of the process of humanization of assistance, that requires the training of professionals to identify and meet their needs. Given the above, this study seeks to address some considerations regarding the interdisciplinary work of assisting the patients´ families in the Adult Intensive Care Unit of Cancer Hospital I, as an efficient proposal with respect to attention to such clinical information requirements, as well as social, medical and/or psychological guidance
  • Corpo e finitude: a escuta do sofrimento como instrumento de trabalho em instituição oncológica Artigo

    Partindo do cotidiano da assistência a pacientes que chegam à Clínica da Dor do Instituto Nacional de Câncer José Alencar Gomes da Silva (INCA), o artigo procura examinar as consequências que traz, para o psíquico, o fato dele estar indissociavelmente ligado ao corpo. Quase sempre afetada profundamente pela doença, a concepção do corpo próprio traz alterações importantes quanto à identificação do sujeito, acarretando disfunções psíquicas, que não só causam sofrimento, como comprometem o tratamento oncológico. Desenvolvendo a conceituação psicanalítica do corpo, ressalta a incidência da linguagem e da fala em sua constituição, que não coincide com a do corpo biológico. Em seguida demonstra que a escuta da fala do paciente, por parte do profissional, é um instrumento de trabalho fundamental em instituição oncológica. Conclui por caracterizar duas posições possíveis a serem ocupadas por aquele que lida com a morte e com a finitude: poupar-se a si próprio do encontro com a dimensão finita e perecível da vida, sentindo pena do paciente, resignando-se e demitindo-se desse encontro; ou escutá-lo com compaixão, reconhecendo o inexorável comum a todos, de modo a que o sujeito possa sofrer isso não tão sozinho e venha a elaborar na palavra algo do horror que atravessa.

    Based on the day-to-day care of patients in the Pain Clinic of a Brazilian cancer hospital (INCA), this article seeks to examine the consequences upon the psychic dimension of the fact of pain being intimately linked to the body. Almost always profoundly affected by the illness, the concept of the subject's own body deeply modifies this identification. This not only causes suffering, but also prejudices oncological treatment. Conceptualizing the body from a psychoanalytical standpoint, this article emphasizes the importance of language and the spoken word in its constitution, as the body perceived by psychoanalysis does not coincide with the biological body. The importance of listening to what the patients say is therefore seen as an important tool in the work of professionals in an oncological institution. Two possible positions regarding professionals dealing with the imminence of death and the finitude of life are then outlined. The first is refraining from being affected by the finite and perishable dimension of life, feeling pity for the patients, resigning and distancing oneself from their predicament. The second involves listening to the patients with compassion, acknowledging the inexorable finitude common to all, such that they may not suffer alone and share some of the horror they are experiencing.
  • Cuidados paliativos: a comunicação como estratégia de cuidado para o paciente em fase terminal Artigo

    Os cuidados paliativos compreendem uma abordagem de assistência ao paciente sem possibilidades de cura e sua família, com o objetivo de proporcionar-lhes uma melhor qualidade de vida, a partir, essencialmente, de uma boa comunicação, sobretudo no campo do cuidado ao paciente terminal. O estudo teve como objetivo averiguar como enfermeiros utilizam a comunicação, no âmbito dos cuidados paliativos, ao assistir o paciente em fase terminal. Trata-se de uma pesquisa exploratória, de natureza qualitativa, na qual participaram 28 enfermeiros atuantes em unidades de internação de um hospital da cidade de João Pessoa (PB), no período de agosto a outubro de 2012. Para a coleta dos dados utilizou-se um formulário. Os dados foram analisados mediante a técnica de análise de conteúdo. Da análise do material, emergiram três categorias: "cuidados paliativos e comunicação - relação interpessoal do enfermeiro com o paciente terminal"; "comunicação em cuidados paliativos como estratégia para fortalecimento do vínculo entre enfermeiro e paciente terminal"; e "importância da comunicação entre enfermeiro e família do paciente terminal sob cuidados paliativos." Conclui-se que a comunicação se configura como um elemento eficaz do cuidado com o paciente em fase terminal e é de suma importância para a promoção dos cuidados paliativos.

    Palliative care involves an approach in the field of care for terminal patients and their families that seeks to assure them better quality of life by establishing good communication. The scope of this study was to verify how nurses use communication in the field of palliative care when assisting patients in the terminal phase. This is exploratory research of a qualitative nature in which 28 nurses working in wards of a hospital in the city of Joao Pessoa in the State of Paraíba participated in the period from August to October 2012. A form was used for data collection that was then analyzed using the content analysis technique. Three categories emerged from the analysis of the material: "palliative care and communication - interpersonal relationship between the nurse and the terminal patient"; "communication in palliative care as a strategy for strengthening the bond between the nurse and the terminal patient"; and "the importance of communication between the nurse and the family of the terminal patient under palliative care." The conclusion reached was that communication is seen to be an effective element of care for the patient in the terminal phase and it is extremely important for the promotion of palliative care.
  • Estudio cualitativo a pacientes de cáncer con soporte paliativo en atención hospitalaria

    Este artículo se centra en el enfermo con cáncer en situación terminal. Trata de los cuidados que requiere la persona que tras haber realizado un recorrido hospitalario en búsqueda de salud es considerada incurable y dirigida a cuidados paliativos (CP) para procurarle bienestar y ayudarle a morir. En la geografía española estos enfermos son atendidos en unidades o servicios de cuidados paliativos (SCP). El objetivo principal en cuidados paliativos es cuidar. Cuidar es aliviar y acompañar al paciente, y dar soporte a los familiares durante el proceso de salud/enfermedad/atención/ muerte del paciente. En el cuidado es importante tratar el cuerpo enfermo. Por lo que prevalece aquello que acontece al afectado y que queda inscrito en su cuerpo. De la misma manera que en la enfermedad incurable el dolor y el sufrimiento marcan un itinerario y trazan unas huellas perceptibles en el mismo. Estos signos pueden resultar una señal que le alerta de la proximidad ala muerte. Aunque los avances científicos han influido, positivamente, en el control del dolor, en algunos casos existen momentos críticos en que se producen situaciones de descontrol y desesperación de quienes lo padecen.

    This article focuses on patients with cancer during the terminal phase. It deals with the care that individuals need after trying to seek health solutions in different hospitals when the disease is deemed incurable and they then require palliative care (PC) to obtain welfare and assist them to die. In the Spanish health system these patients are cared for in palliative care units or services. The main objective of palliative care is to give comfort. Comforting is destined to alleviate and support patients and families during the process of health / disease / care / patient death. During this period, it is important to care for the ailing body. Similarly, when a disease is incurable, the pain and suffering runs its inexorable course and leaves perceptible traces. These signs can be an indication that death is nigh. Although scientific advances have produced positive results in the control of pain, in some cases there are critical moments when situations of misery and despair arise for the terminal patients.
  • Cuidados paliativos: entre la humanización y la medicalización del final de la vida

    El artículo analiza la atención de pacientes terminales en servicios de cuidados paliativos, explorando las tensiones entre la humanización del cuidado y el avance de la medicalización en el final de la vida. La investigación en que se apoya el artículo sigue una estrategia metodológica cualitativa, basada en entrevistas con profesionales y pacientes y en observaciones etnográficas en un servicio de cuidados paliativos de la Ciudad de Buenos Aires, Argentina. El artículo describe la configuración de un cuidado personalizado e integral, base de la propuesta de la disciplina de brindar una atención más "humana" en el final de la vida. Asimismo, se analiza la forma en que éstas prácticas están mediadas por el avance de procesos de medicalización, en las que el cuidado es considerado menos un aspecto "no profesional" de la práctica médica que un área de atención sobre la que se afirman habilidades y saberes técnicos específicos. El artículo evidencia cómo las lógicas del cuidado y de la medicalización, más que lógicas opuestas, funcionan de forma complementaria.

    This paper analyzes the palliative care of terminal patients, examining the tensions between the humanization of care and the progressive medicalization at the end-of-life situation. The research upon which the article is based adopts a qualitative methodological approach derived from interviews with professionals and patients and ethnographic observations in a palliative care unit in the City of Buenos Aires, Argentina. The article describes the configuration of personalized and comprehensive health care based on the core values of more humanized end-of-life care promoted by palliative care supporters. Similarly, the paper analyzes how these practices are assisted by progress in the medicalization process in which the dimension of care is considered less an unprofessional area of medical practice than an area of care in which specific technical skills and know-how are employed. The articles explores how instead of being divergent, the logic of care and medicalization work in a complementary fashion.
  • O trabalho de cuidar de pacientes terminais com câncer Artigo

    Estudo realizado na unidade de cuidados paliativos do Instituto Nacional de Câncer José Alencar Gomes da Silva (INCA), com o objetivo de compreender as características do trabalho e suas repercussões na saúde dos profissionais. Adotou-se abordagem de cunho qualitativo, com base em observação participante e entrevistas individuais semiestruturadas com profissionais do setor de internação hospitalar. A maioria das profissionais é do sexo feminino e desconhecia a proposta dos cuidados paliativos quando começou a trabalhar na unidade. O trabalho em equipe é muito valorizado pelas profissionais que envolvem a família nas ações de cuidado e incentivam a participação de pacientes e familiares na tomada de decisões. As trabalhadoras vivenciam elevado desgaste emocional ao conviverem com o sofrimento de pacientes e familiares e desgaste físico pela grande demanda provocada pela rotina cotidiana e pela dependência da maioria dos pacientes: é elevado o número de licenças que recebem por doenças osteomusculares e psiquiátricas. Há necessidade de programas destinados a promover a saúde dessas profissionais e que estejam alertas a cuidados cotidianos para evitarem os desgastes. Entretanto, suas sugestões de melhoria das condições de trabalho e saúde foram enviadas apenas à instituição para que tome providências.

    This is a study conducted in the palliative care unit of the INCA National Cancer Institute, in an attempt to understand the nature of the work and its effects on the health of the professionals. A qualitative approach is adopted, based on participant observation and semi-structured interviews with professionals from the hospital. Most of the professionals are female and were unaware of the concept of palliative care when they started their careers in the Unit. Teamwork is highly valued by the professionals who involve the families in the care actions and motivate the participation of patients and relatives in decision-making. However, these workers experience high emotional stress due to their involvement in the suffering of patients and families. They are also exposed to considerable physical stress caused by the intense demand of the daily routine and the dependence of the majority of patients. The study revealed that there are many professionals on leave of absence due to musculoskeletal problems and psychiatric disorders. There is a pressing need for health support programs for these professionals and awareness of daily care measures for their own health. However, their suggestions for improvement of working and health conditions are merely passed on to be implemented by the institutions themselves.
  • Cuidados paliativos em pacientes com HIV/AIDS: princípios da bioética adotados por enfermeiros Artigo

    Neste estudo, objetivou-se investigar os princípios da Bioética considerados pelos enfermeiros inseridos na pesquisa ao assistir o paciente com HIV/Aids sob cuidados paliativos. Estudo exploratório, com abordagem qualitativa, realizado com doze enfermeiros em um Hospital público da cidade de João Pessoa (PB). Para a coleta dos dados, utilizou-se um formulário. O material empírico foi analisado qualitativamente, por meio da técnica de análise de conteúdo. Da análise qualitativa, emergiram as categorias: Respeito à autonomia do paciente com HIV/Aids sob cuidados paliativos; e, Valorização dos princípios da beneficência, não maleficência e justiça na prática dos cuidados paliativos ao paciente com HIV/Aids. Os resultados mostraram que os participantes da pesquisa reconhecem a importância dos princípios da Bioética na assistência ao paciente com HIV/Aids sob cuidados paliativos. O trabalho traz uma reflexão e suscita a realização de novos estudos que possam contribuir para melhorar a qualidade de vidas desses pacientes com uma prática pautada em princípios éticos.

    This study sought to investigate the principles of bioethics considered by nurses involved in research while caring for patients with HIV/Aids under palliative care. An exploratory study, with a qualitative approach, was conducted with twelve nurses in a public hospital in the municipality of João Pessoa in the State of Paraíba. A form was used for data collection and the empirical material was analyzed qualitatively using the content analysis technique. The following categories emerged from the qualitative analysis: Respect for the autonomy of HIV/Aids patients under palliative care; and Enhancement of the principles of charity, non-malevolence and justice in providing palliative care to HIV/Aids patients. The results revealed that the participants acknowledge the importance of bioethical principles while caring for patients with HIV/Aids under palliative care. The work provides a reflection on and elicits the pursuance of further studies which can contribute to improve the quality of life of these patients with care based on ethical principles.
  • Luto antecipatório em pacientes com indicação para o Transplante de Células-Tronco Hematopoéticas Artigo

    O objetivo deste estudo foi compreender como o processo de luto antecipatório se instala em pacientes a serem submetidos ao Transplante de Células Tronco-hematopéticas (TCTH). Foi realizado um estudo transversal, na abordagem clínico-qualitativa. Amostra de 17 pacientes, a maioria mulheres casadas, entre 20 e 42 anos de idade e diagnóstico de Leucemia Mieloide Crônica. Dados coletados por um roteiro de entrevista semiestruturada, aplicado individualmente e submetidos à análise de conteúdo temática. Os resultados indicam que a perda da saúde impõe um luto em uma história de vida já permeada por privações e perdas prematuras. Constatou-se que as reações esperadas frente ao luto normal foram expressas pelos participantes e que a estratégia de enfrentamento privilegiada foi o apego à fé. Os planos futuros envolviam alcançar a cura, retorno à normalidade e reinserção profissional. Os resultados podem auxiliar as equipes multiprofissionais a compreenderem as implicações emocionais do adoecimento/tratamento para implantação de estratégias preventivas e interventivas. Como pontos críticos, a equipe deve atentar para a forma de comunicar o diagnóstico e a possibilidade de traçar um plano terapêutico que mantenha vivo o espírito de luta do paciente, reforçando o elo de confiança com os profissionais de saúde.

    The scope of this study was to understand how the process of anticipated grieving is imbued in patients undergoing Hematopoietic Stem Cell Transplantation (HSCT). A cross-sectional clinical-qualitative study was conducted on a sample of 17 patients, mostly women, married, aged between 20 and 42 years and diagnosed with Chronic Myeloid Leukemia. Data was collected by semi-structured interviews applied individually and subjected to thematic content analysis. The results indicate that the loss of health imposes a new challenge in a life history already permeated by great hardships and premature losses. It was found that the expected reactions faced with normal grieving were expressed by the participants and that the most prevalent coping strategy was holding steadfast to their faith. Future plans involved being healed, returning to normality and vocational rehabilitation. The results may help the multidisciplinary teams to understand the emotional implications of the illness/treatment for implementing both preventive and intervention strategies. The critical aspect is that staff must be attentive as to how to communicate the diagnosis and the possibility of outlining a therapeutic plan, in order to augment the fighting spirit of the patient and strengthen the bond of trust with health professionals.
  • Cuidados paliativos: uma abordagem a partir das categorias profissionais de saúde Artigo

    O Cuidado Paliativo surge como uma filosofia humanitária de cuidar de pacientes em estado terminal, aliviando a sua dor e o sofrimento. Estes cuidados prevêem a ação de uma equipe interdisciplinar, onde cada profissional reconhecendo o limite da sua atuação contribuirá para que o paciente, em estado terminal, tenha dignidade na sua morte. Este artigo trata a questão da morte e do morrer, tanto na visão tradicional como na contemporaneidade, e como o cuidado paliativo tem sido tratado nas categorias de trabalho de medicina, serviço social, psicologia e enfermagem. A metodologia deste trabalho consiste na revisão bibliográfica de artigos localizados na base de dados Scielo, revistas eletrônicas e livros técnicos relacionados com o tema. A análise dos artigos apontou para uma carência de disciplinas que tratem da temática da morte nos currículos profissionais, para poucos serviços de cuidados paliativos na sociedade brasileira e para barreiras que se colocam a esse novo olhar ao paciente terminal. Esta pesquisa visa ampliar a discussão dos cuidados paliativos na saúde pública, e fornecer subsídios a futuros estudos que tratarão da temática.

    Palliative care has emerged as a humanitarian philosophy of caring for terminally ill patients, alleviating their pain and suffering. This care involves the action of an interdisciplinary team, in which all the professional recognize the limits of their performance will help the terminally ill patient to die with dignity. This article deals with the issue of death and dying, both from the traditional and the contemporary standpoint, and how palliative care have been treated in the job categories of medicine, social work, psychology and nursing. The methodology of this study consists of a literature review of articles in the SciELO database, electronic journals and technical books related to the topic. Analysis of the articles revealed a shortage of subjects that deal with the theme of death in professional curricula, as well as few palliative care services in Brazilian society and barriers faced by this new approach to the terminal patient. This research aims to broaden the discussion of palliative care in public health, and provide information for future studies that will address the theme.
  • Percepção dos enfermeiros sobre o significado dos cuidados paliativos em pacientes com câncer terminal Artigo

    Este estudo teve como objetivo conhecer a percepção do enfermeiro diante do paciente com câncer sob cuidados paliativos. Trata-se de um estudo exploratório com abordagem qualitativa, realizado com enfermeiros assistenciais de um hospital que atende pacientes oncológicos em regime de cuidados paliativos, situado no município de João Pessoa (PB). Participaram do estudo nove enfermeiros, que atuavam no referido hospital. O material empírico foi coletado utilizando-se a técnica de entrevista semiestruturada e analisado mediante a técnica de análise de conteúdo. A análise interpretativa das entrevistas possibilitou a construção de três categorias: Promoção da qualidade de vida através do alivio da dor e sofrimento; Cuidados Paliativos: um olhar multiprofissional para o paciente terminal e família no processo de luto; Comunicação: fonte de dignidade no processo de terminalidade. Conclui-se que o estudo possibilitou evidenciar que os enfermeiros envolvidos reconhecem a importância da equipe multiprofissional, propiciando ao enfermeiro reflexões acerca do uso da comunicação como elemento essencial do cuidar para paciente e família sob cuidados paliativos. Espera-se que os dados obtidos possam subsidiar novas investigações sobre a temática.

    This study sought to assess the perception of nurses with respect to cancer patients under palliative care. It is an exploratory study with a qualitative approach conducted with nurses from a hospital attending cancer patients under palliative care located in the city of João Pessoa, State of Paraíba. The study included nine nurses who worked in the hospital. The empirical material was collected using the technique of semi-structured interviews and analyzed using the content analysis technique. The interpretative analysis of the interviews led to the definition of three categories: Improving the quality of life through the alleviation of pain and suffering; Palliative Care: a multi-professional study of terminal patients and their families in the grieving process; Communication: a source of dignity in the terminal care process. The conclusion reached is that the study revealed that the nurses involved acknowledge the importance of the multidisciplinary team. It enables the nurses to reflect on the use of communication as an essential element of care for patient and family under palliative care. It is hoped that the data obtained may foster further research on the topic.
  • Concepções da equipe multiprofissional sobre a implementação dos cuidados paliativos na unidade de terapia intensiva Artigo

    Este estudo objetivou analisar as concepções da equipe multiprofissional sobre a implementação de cuidados paliativos em uma unidade de terapia intensiva adulto. Trata-se de uma pesquisa exploratório-descritiva, com abordagem qualitativa, realizada com 14 profissionais de saúde de um hospital público de ensino. Os dados foram coletados entre fevereiro e abril de 2012, através de entrevista semiestruturada e observação não participante e interpretados pela análise de conteúdo. Foram identificadas três categorias temáticas: Assistir o paciente terminal em UTI promovendo o conforto físico; Despreparo da equipe em lidar com o paciente terminal; e Desafios na prática dos cuidados paliativos no ambiente da terapia intensiva. Os entrevistados referiram conhecer parcialmente a proposta dos cuidados paliativos e na prática assistencial observam-se divergências nas condutas terapêuticas da equipe, demonstrando falta de interação e de comunicação entre os profissionais. Faz-se necessário a elaboração de uma política nacional que respalde o cuidado ao paciente crítico terminal, a educação permanente/continuada dos profissionais e a criação de protocolos assistenciais para promoção do conforto do paciente durante a fase final da vida e de sua família.

    The scope of this paper was to analyze the perceptions of the multi-professional team on the implementation of palliative care in an adult intensive care unit. An exploratory-descriptive study using a qualitative approach was conducted with 14 health professionals from a public teaching hospital. The information was collected between February and April 2012, by means of semi-structured interviews and non-participatory observation interpreted using content analysis. Three thematic categories were identified: Care for terminal patients in an ICU fostering physical comfort; Lack of preparation of the team in dealing with terminal patients; and Challenges of palliative care practices in the intensive care environment. The interviewed parties reported having some knowledge of the proposal for palliative care though divergences were observed in the therapeutic conduct of the team in the care provided, demonstrating a lack of interaction and communication among the professionals. The drafting of a national policy to promote care for terminally ill patients is necessary, as well as ongoing training of professionals and the creation of care protocols for promoting the comfort of the patients and their families during the end of life phase.
  • Notícias difíceis: sentidos atribuídos por familiares de crianças com fibrose cística Artigo

    Este artigo apreendeu sentidos atribuídos por familiares de crianças com fibrose cística sobre as notícias difíceis transmitidas por profissionais de saúde durante o tratamento num hospital público terciário da zona sul do Rio de Janeiro. Resulta de pesquisa qualitativa e análise de dez narrativas colhidas em dois meses de 2010. Supôs-se que esses sentidos são pouco conhecidos, interferem na vivência e enfrentamento do processo de adoecimento e cuidado dos filhos. Evidenciou-se que o conteúdo da notícia era percebido diferentemente segundo: a clareza com que ela era adequada a cada familiar, a forma direta de transmiti-la sem preparação do ouvinte, a consideração do contexto em que viviam as pessoas e as consequências que nele provocaria; a presença de um acompanhante no momento da notícia e a fragmentação das informações. Os resultados indicaram a necessidade de sensibilizar profissionais para aspectos comunicacionais nos encontros clínicos e inclusão dessa capacitação na graduação e/ou ao longo da atividade profissional. Viu-se que a notícia, embora não parecesse difícil para os profissionais podia sê-lo para familiares; que ela afeta a experiência da doença e que os familiares esperam dos profissionais equilíbrio entre eficiência técnica e competência relacional.

    This article studied some of the meanings that difficult news assumes when transmitted by health professionals to relatives of young cystic fibrosis patients undergoing treatment at a public hospital located in the south zone of the city of Rio de Janeiro, Brazil. It is the result of qualitative research and analysis of ten narratives collected during two months in 2010. These meanings are not understood by professionals and interfere with the evolution of the illness, the coping process and the care given to the children. It has been observed that the content of the news is perceived differently depending on: The clarity with which it is transmitted to each family member; The forthright way it is conveyed without the preparation of the listener; The consideration of the context in which people live and the consequences involved; The presence of a family member when the news is delivered; and the fragmentation of information. Results indicate the need to make professionals aware of the importance of communication aspects in clinical meetings and the inclusion of this training process in the medical curriculum as well as during physicians' professional lives. It seems that in delivery of the news the family expects a balance between technical efficiency and conversational tact from professionals.
  • Percepção de familiares e profissionais de saúde sobre os cuidados no final da vida no âmbito da atenção primária à saúde Artigo

    O objetivo deste estudo é refletir sobre os cuidados às pessoas com doenças em fase terminal na atenção primária à saúde (APS). Esta pesquisa torna-se relevante no contexto atual de transição demográfica e epidemiológica, na qual há um crescente envelhecimento populacional e um aumento da prevalência de doenças crônico-degenerativas. Por meio de pesquisa qualitativa sob o enfoque hermenêutico-crítico, foram realizadas entrevistas individuais com sete familiares, três profissionais da Estratégia Saúde da Família (ESF) e dois profissionais do Programa de Assistência Domiciliar. A análise ressaltou duas categorias empíricas: cuidados no final da vida na percepção dos familiares; e na dos profissionais de saúde. Os resultados evidenciaram que, apesar do discurso de humanização, refletida na prerrogativa de viver os últimos momentos de vida com a família, esconde-se um grave problema de descontinuidade dos cuidados. Concluiu-se que, apesar dos esforços, pouco se logra de melhoria na qualidade de vida de quem morre por uma doença em fase terminal no domicílio, sob os cuidados do Sistema Único de Saúde (SUS) em municípios distantes dos grandes centros.

    The objective of this study is to reflect on care for people with terminal illnesses in primary health care. This study is relevant in the current context of demographic and epidemiological transition, in which there is an increase in the aging population and a rise in the prevalence of chronic degenerative diseases. By means of qualitative research with a critical hermeneutic focus, individual interviews were conducted with seven family members, three professionals from the Family Health Strategy and two professionals from the Home Care Program. The analysis established two empirical categories: end of life care from the perception of family members and that of health professionals. The results showed that, despite the humanization discourse, reflected in the prerogative of living the final moments of life with the family, a serious problem in the discontinuity of care is concealed. It was concluded that, despite the efforts, little is achieved in improving the quality of life for those who die from terminal diseases at home in the Unified Health System in municipalities distant from large centers.

    Este estudo teve como objetivo investigar o significado da morte para médicos que atuam com pacientes em condição crítica no contexto do Transplante de Medula Óssea (TMO). A pesquisa foi fundamentada no enfoque metodológico qualitativo e no estudo de caso coletivo, e utilizou o referencial teórico da psicodinâmica do trabalho. Participaram os cinco médicos vinculados a uma Unidade de TMO de um hospital universitário do interior paulista. Aplicou-se um roteiro semiestruturado, que abordava as vivências pessoais, acadêmicas e profissionais suscitadas pela situação de terminalidade. Os dados foram sistematizados nas categorias: "Trabalho e identidade profissional: a peleja do médico contra a morte" e "Vocação e formação médica: tudo começa em casa". Os resultados mostram que, no âmbito médico-hospitalar, a morte é quase sempre reduzida à racionalidade tecnicista e por isso deve ser combatida a qualquer custo. As significações atribuídas à morte são ancoradas nos valores da sociedade contemporânea e subjetivadas nos percursos e percalços enfrentados pelo médico ao longo de sua história de vida e na formação acadêmica, que influenciam o modo como os profissionais vivenciam o processo do morrer dos pacientes.

    This study sought to investigate the significance of death for doctors who work with terminally ill patients in the Bone Marrow Transplant (BMT) context. The research was based on a qualitative, methodological and collective case study approach, and used Dejours' psychodynamics of work as a theoretical framework. Five practitioners linked to a Bone Marrow Transplant Unit in a university hospital in São Paulo, Brazil, participated in the research. A semi-structured script was applied which addressed the personal, academic and professional experiences elicited by a terminal situation. Data were organized into two categories: "Work and professional identity: the doctor's battle against death" and "Vocation and medical training: it all beggins at home." The results show that, in the medical and hospital context, death is almost always reduced to technical rationality and this must be avoided at all costs. The significances attributed to death are rooted in the values of contemporary society and subjectively perceived through the physician's life trajectory and academic training, which influence how these professionals experience the dying process of the patients.

    O envelhecimento populacional e a falta de cuidadores resultam em uma crescente busca pela institucionalização. A convivência com a morte faz parte do cotidiano de trabalho dos profissionais de saúde, causando-lhes sobrecarga emocional. Objetivou-se conhecer a vivência dos profissionais de saúde atuantes em uma instituição de longa permanência para idosos diante do processo de morrer e de morte. Utilizaram-se os referenciais da Teoria Fundamentada nos Dados e do Interacionismo Simbólico, respectivamente, metodológico e teórico. Foram entrevistados 20 profissionais de saúde, com formação variada. O fenômeno central dos resultados foi: reconstruindo formas para lidar com as circunstâncias de morrer e morte. Originaram-se as categorias: compreendendo a morte como parte da existência humana; buscando adquirir conhecimentos para enfrentar os episódios de morrer e morte; refletindo sobre a própria morte. Sem um terreno propício para o desenvolvimento sistemático do tema, as interações entre os trabalhadores com a troca de experiências sobre morte e morrer permanecerá restrita a um nível subjetivo. Na conclusão, é enfatizada a importância de uma mudança (metamorfose) no contexto institucional e na educação em saúde, com foco mais específico na tanatologia.

    Population aging and a shortage of caregivers result in a growing demand for institutionalization, and finitude is one the reflections that permeates aging. Living with death is part of the daily routine of health professionals resulting in emotional overload. This study sought to ascertain the experiences of the process of dying and death of health care professionals in a long-stay institution for the elderly. Methodological and theoretical Grounded Data Theory and Symbolic Interaction were used, respectively. Twenty health professionals from varied backgrounds were interviewed. The core topic of the results was: reconstructing ways to deal with the circumstances of dying and death. This led to the following categories: seeing death as part of human existence; seeking to acquire knowledge to handle cases of death and dying; reflecting on one's own death. Without fertile ground for the systematic examination of the topic, the interaction among workers about the exchange of experiences on death and dying remain restricted to the subjective level. In the conclusion, the importance of a metamorphosis in the institutional context and in health education, with more specific focus on thanatology, is emphasized.
  • Conhecimento, envolvimento e sentimentos de concluintes dos cursos de medicina, enfermagem e psicologia sobre ortotanásia Artigo

    Na ortotanásia ocorre a suspensão dos procedimentos médicos na fase terminal do paciente para que ocorra a morte natural, com o alívio dos sintomas que levam ao sofrimento. Neste processo, profissionais como médicos, enfermeiros e psicólogos, interagem com o paciente e seus familiares. Por isso, é desejável que desde a graduação estes profissionais tenham em suas formações disciplinas voltadas para esta demanda. O objetivo desta pesquisa qualitativa foi avaliar o conhecimento sobre ortotanásia dos concluintes dos cursos de medicina, enfermagem e psicologia de uma universidade. Por meio de questões disparadoras foi realizada entrevista semiestruturada com 22 alunos. As entrevistas foram gravadas e transcritas para análise de conteúdo e identificação de categorias temáticas. Foram identificadas três categorias: conhecimento sobre a ortotanásia, quem deveria se envolver com este processo e sentimentos vivenciados frente à morte. Os dados mostraram que os estudantes possuem um baixo conhecimento sobre o tema, consideram importante o envolvimento dos familiares na decisão da ortotanásia, e não se sentem preparados para o enfrentamento da morte. Conclui-se que há a necessidade de mudanças sobre o enfoque dado ao tema terminalidade nos cursos de graduação da área da saúde, visando preparar melhor os futuros profissionais.

    Orthothanasia involves the suspension of medical procedures for terminal phase patients, which leads to a natural death, relieving the symptoms that cause suffering. In this process, professionals such as physicians, nurses and psychologists, interact with patients and their families. Therefore, it is desirable that during undergraduate studies these professionals should take subjects geared to handle this aspect. The scope of this qualitative study was to evaluate the awareness with respect to orthothanasia of undergraduates in medicine, nursing and psychology courses in a university. Trigger questions in semi-structured interviews were conducted with 22 students. The interviews were recorded and transcribed for content analysis and core identification themes. Three categories were identified: knowledge about orthothanasia; who should be involved in this process; and feelings experienced when facing death. The data revealed that students have scant knowledge about the subject, consider the family involvement in the orthothanasia decision to be important and they do not feel prepared to deal with death situations. The conclusion points to the need to change the focus on the end-of-life issue in the undergraduate courses in the area of health care in order to prepare the future professional adequately.
  • A construção da "boa morte" em diferentes etapas da vida: reflexões em torno do ideário paliativista para adultos e crianças Artigo

    Este artigo aborda o ideário dos Cuidados Paliativos, para uma trajetória na direção de uma "boa morte", para adultos e crianças. A partir do exame de livros-texto, manuais e artigos, de observação de cursos e congressos da especialidade, voltada a pacientes diagnosticados como "fora de possibilidades terapêuticas de cura", são examinadas as diferenças e semelhanças entre o atendimento a adultos e crianças. A equipe busca prestar uma assistência à "totalidade bio-psico-social-espiritual" do doente e de seus familiares, para propiciar "qualidade de vida" no tempo restante, com autonomia do enfermo e controle de seus sintomas. Segundo o modelo paliativista, os atores sociais envolvidos nos cuidados devem aceitar o término da vida. Trata-se de produzir uma "boa morte", "com dignidade", pacífica, tranquila, aceita, visível e compartilhada socialmente, com suporte da equipe multiprofissional. Tal configuração é complexa, pois diversos fatores e circunstâncias podem interferir, sobretudo no caso de crianças, quando se apresenta um paradoxo, uma vez que a ocorrência de doença terminal nesta etapa da vida - tão valorizada na cultura ocidental contemporânea - constitui um drama social.

    This article deals with the vision of Palliative Care leading to a "good death" for adults and children. The differences and similarities between the care of adults and children are examined based on the scrutiny of textbooks, manuals and articles, as well as the observation of courses and congresses within the specialty, which focus on patients diagnosed as "beyond therapeutic cure possibilities." Health teams seek to provide care for the "bio-psycho-social-spiritual totality" of the patients and their family members, to offer "quality of life," with full autonomy and control of their symptoms. In accordance with the palliative care model, the social actors involved in care must accept the end of life inevitability. It is a question of ensuring a "good death," "with dignity," peaceful, accepted, transparent and socially shared, with the support of a multi-professional team. It is a complex configuration, as various factors and circumstances come into play. This is especially true in the case of children, when a paradox arises, since terminal illness at this stage of life - so highly valued in contemporary Western culture - constitutes a social drama.
  • Perda e luto: vivências de mulheres que interromperam a gestação por malformação fetal letal Artigo

    O presente estudo teve por objetivo conhecer as vivências de luto de mulheres que interromperam a gestação sob autorização judicial, devido à malformação fetal incompatível com a vida. Participaram do estudo dez mulheres atendidas no Setor de Medicina Fetal do Hospital das Clínicas de Botucatu. Para coleta dos dados foi realizada entrevista semiestruturada quarenta dias após a interrupção. As entrevistas foram audiogravadas, transcritas na íntegra e tiveram os dados analisados na perspectiva da análise temática de conteúdo. Os resultados revelaram que as mães buscaram explicações e significados para a perda, sendo muito frequentes respostas religiosas e autoculpabilizantes. Os relatos marcaram sentimentos de tristeza, saudade e sensação de vazio pela perda do filho, revelando também a necessidade das mães de manterem-se ligadas a ele. As mães estavam e continuaram vinculadas aos seus filhos e a interrupção da gestação, embora tenha sido uma escolha para minimizar a dor de uma perda inevitável, não as poupou de vivências de grande sofrimento. O estudo traz subsídios para a discussão e planejamento de abordagens e cuidados com a saúde de mulheres que interrompem a gestação por malformação fetal letal, mediante autorização judicial.

    The scope of this study was to investigate the grieving experiences of women who terminated pregnancies under judicial authorization, due to life-incompatible fetal malformation. Ten women attended in the Fetal Medicine Department of Botucatu Clinical Hospital participated in the study. Data collection was conducted by means of semi-structured interviews forty days after termination. The interviews were recorded and transcribed in full, with the data analyzed from the thematic content analysis perspective. The results revealed that the mothers sought explanations and meanings for the loss, with religious responses and self-blame being very frequent. The reports were marked by feelings of sadness, longing and sensations of emptiness due to the loss of the child, revealing the need of the mothers to dwell on the issue. The mothers were and continued to be linked to their children; the termination of the pregnancy, although being a choice to minimize the pain of an inevitable loss, did not spare the women from experiences of great suffering.>The study includes input for the discussion and planning of health approaches and care for women who terminate their pregnancy due to lethal fetal malformation, by means of judicial authorization.

    O estudo teve como foco a vivência de mulher jovem com câncer de mama e objetivou compreender o adoecer, o morrer e a morte dela, a partir dos sentidos atribuídos aos eventos de sua vida. Apóia-se na ideia de que se morre a partir do que foi vivido e no modo com que este vivido vai sendo ressignificado ao longo do tempo. Trata-se de pesquisa qualitativa que se configura como um estudo de situação, o qual permitiu construir a Linha da Vida de Beth através do emprego de sua História de Vida em que se destaca suas narrativas, eventos considerados, por ela, marcantes e que conformaram o seu modo de viver e, nele, o morrer. Além da descrição minuciosa de como se deram os encontros de entrevista, também discute-se duas categorias centrais do estudo, denominadas de "Morrer a vida - as pequenas mortes vivenciadas por Beth" e "Beth vivendo seu morrer e morte". O estudo enfatiza a necessidade dos profissionais da saúde tomarem para si a responsabilização por um modo de cuidado que não vise tão somente a cura, mas que tenha como foco a dimensão humana com o objetivo de prover o bem estar e a manutenção da vida, enquanto esta for possível.

    This study focused on the experience of a young woman with breast cancer and sought to understand the progression of her falling ill, dying and death based on the significance attributed to the events of her life. It concentrates on the idea that one dies from how one has lived and in the way that a new meaning is attributed to this life over the course of time. It is qualitative research that is presented as a situational study that enabled the construction of Beth's Life Line using her Life History in which, judging from her narratives, events considered significant by her and that confirmed her way of living, stand out and are reflected in her death. Besides the detailed description of how the interview meetings occurred, it also discusses two central categories of the study, called "Ebbing of life - the small deaths experienced by Beth" and "Beth living out her dying and death." The study emphasizes the need for the health professionals to assume responsibility for a kind of care that does not only seek a cure, but also focuses on the human dimension, aiming to ensure well-being and maintenance of life, while this is still possible.
  • La muerte como proceso: una perspectiva antropológica

    Este artículo, desde una perspectiva antropológica, combina recientes aproximaciones que cuestionan la homología entre muerte y deceso biológico, con la tradición de estudios que revisan los rituales de muerte en el área geográfica andina. En particular, examina cómo se produce en El Cajón, Catamarca, noroeste argentino, la incorporación de un fallecido al esquema cosmológico, y cómo la muerte, que es un puntapié inicial para ello ocurra, no es meramente un evento biológico sino también un proceso social de gran dinamismo. A partir del trabajo de campo etnográfico, y tomando como herramientas metodológicas entrevistas abiertas de sesiones múltiples y observación participante, presenta una propuesta analítica que procura superar la homología entre muerte y deceso biológico, sugiriendo una noción que incluye la dimensión orgánica, pero también la social: el proceso de la muerte.

    This article combines recent approximations that question the homology between death and biological demise from an anthropological perspective with the tradition of studies that review the death rituals in the Andean geographical area. In particular, it examines how in El Cajón, Catamarca, Northeast Argentina the dead are incorporated in the cosmologic scheme, and how death, being the initial step for this to occur, is not merely a biological event but also a highly dynamic social process. Based on ethnographical field work and using multiple-session open interviews and participant observation as methodological tools, it presents an analytical proposal that seeks to transcend the homology between death and biological demise, suggesting a notion that includes not only the organic dimension but also the social dimension: the process of death.

    O artigo se propõe a refletir acerca da legalidade da manifestação antecipada de vontade, instituída no Brasil por meio da Resolução nº. 1.995/12 do Conselho Federal de Medicina, em face da omissão legislativa e considerando a possível vinculação dos médicos e familiares de pacientes terminais ao Testamento Vital. Analisa a constitucionalidade dessas diretivas e, por conseguinte, a constitucionalidade da própria Resolução à luz do novo paradigma constitucional brasileiro.

    The article seeks to reflect on the legality of the early manifestation of will, established in Brazil through Resolution No. 1.995/12 of the Federal Council of Medicine in the face of the legislative omission and considering the possible linkage of physicians and relatives of terminally ill patients in drafting the Last Will and Testament. It examines the constitutionality of these policies and, therefore, the constitutionality of the resolution itself in light of the new Brazilian constitutional paradigm.

Todos os medos do ser humano estão, no fundo, relacionados com o da morte. Tudo isso por ela ser um mistério do qual pouco ou nada sabemos, embora seja natural. À vista disso, as religiões dão a este tema o seu principal enfoque. Assim sendo, procuro neste artigo tratar o tabu da morte, refletindo temas relacionados a ele. Temas que estão presentes na vida das pessoas, sobretudo quando passam pela experiência da perda de alguém do seu círculo familiar ou de amizade. Dentre eles, abordo a Unção dos Enfermos, as Exéquias, a Missa do Sétimo Dia e a tradição de rezar pelos mortos. Temas que ajudam a lidar com o tabu da morte ou a perda de um ente querido.

    The more death is studied, the more it remains a mystery. Dealing with death is no easy matter and for that reason it is so frightening. All the fears of human beings are fundamentally related to the fear of death. This is because it is a mystery about which we know little or nothing, although it is natural to die. In view of this, religions give this topic a primary focus. Therefore, in this article an attempt is made to deal with the taboo surrounding death and reflect on issues associated with it. These themes are present in people's lives, especially when they have the experience of losing someone in their family or circle of friends. Among these subjects, the aspects of the Anointing of the Sick, the Funeral Mass, the Seventh Day Mass and the tradition of praying for the dead are all touched upon. These are all themes that help to deal with the taboo of death or losing a loved one.
  • Impacto da morte do filho sobre a conjugalidade dos pais Artigo

    O objetivo deste estudo qualitativo foi compreender a experiência do casal que perdeu um filho acometido por câncer, focalizando o impacto da morte sobre a relação conjugal. A estratégia metodológica utilizada foi o estudo de caso, buscando descrever o processo de elaboração do luto e a repercussão da morte do filho sobre a conjugalidade dos pais. Participou do estudo um casal unido havia 39 anos, que três anos antes perdera o filho mais novo. Os cônjuges foram entrevistados individualmente e, em um segundo momento, conjuntamente. Foram utilizados roteiros de entrevista semiestruturada. Observou-se que a comunidade religiosa da qual o casal faz parte tem sido a principal fonte de apoio na elaboração do luto, embora seus preceitos possam, no caso da esposa, impedir a livre expressão de seu sofrimento. Ainda que o casal tenha permanecido unido, a conjugalidade ficou abalada após a morte do filho. Os cônjuges mostraram dificuldade em discorrer sobre o casamento, sendo que a esposa apontou a ausência do filho como um dos motivos para a mudança ocorrida no relacionamento conjugal. A conjugalidade e a religiosidade/espiritualidade despontaram como dimensões importantes a serem abordadas pelas equipes de saúde no atendimento aos familiares enlutados.

    The scope of this qualitative study was to understand the experience of the couple who lost a child stricken by cancer,focusing on the impact of death on the marital relationship. The methodological strategy used in this paper was the case study, seeking to describe the process of elaboration of mourning and the impact of the son's death on the parents' marital relationship. The couple who participated in the study had been together for 39 years and had lost their youngest son three years earlier. The spouses were interviewed individually and then together. Semi-structured interview scripts were used for this purpose. It was established that the religious community to which the couple belongs has been the main source of support in dealing with the mourning process, although its precepts may prevent the free expression of their suffering, particularly in the wife's case. Although the couple has remained together, the marital relationship was shaken after their son's death. The couple had difficulty in talking about their marriage, though the wife pointed to the absence of their son as one of the reasons for the change in their marital relationship. The marital relationship and religiosity/spirituality emerged as important dimensions to be addressed by health staff in attending bereaved families.
  • Experiência em cuidados paliativos à criança portadora de leucemia: a visão dos profissionais Artigo

    O cuidado paliativo desenvolve-se por meio de assistência multiprofissional com ações de suporte e conforto para a criança e sua família. O suporte se constitui do alívio do sofrimento pelo controle da dor e dos sintomas, bem como pelo apoio psicossocial e espiritual. Trata-se de um estudo de caso qualitativo que objetivou compreender a visão da equipe multidisciplinar perante a criança portadora de leucemia em cuidado paliativo, em uma instituição hospitalar de Belo Horizonte. Os sujeitos da pesquisa foram 17 profissionais de saúde da equipe de assistência direta à criança nesses cuidados. Os dados foram submetidos à técnica de análise de conteúdo. A experiência profissional em cuidado paliativo é intrínseca ao sucesso e à continuidade do tratamento do paciente. Verificou-se que os profissionais possuem pouca experiência nessa área e têm dificuldade em lidar com os sentimentos, se sentindo, às vezes, impossibilitados de agir mediante as angústias dos envolvidos e com a morte. Contudo, apesar de a área de atuação gerar sofrimento e angústia ao profissional, percebe-se que a equipe multiprofissional identifica-se e gosta da área o que é importante para a criança e sua família se sentirem seguros, respeitados e acolhidos pelos profissionais.

    Palliative care is developed through multi-professional assistance with actions to provide support and comfort for the children and their families. The support involves the alleviation of suffering by controlling the pain and its symptoms, as well as ensuring psychosocial and spiritual support. This is a qualitative case study which sought to understand the multidisciplinary view of the team vis-à-vis a child with leukemia in palliative care, in a hospital in Belo Horizonte. The subjects of research were 17 health professionals of the direct assistance team to the child receiving such care and the data were analyzed using the content analysis technique. The professional experience in palliative care is intrinsic to the success and continuity of the patient's care. It was found that the professionals have little experience in this area and have difficulty dealing with their emotions, and sometimes become unable to act when faced with the anguish of those involved and with death. However, despite the fact that the area of activity causes suffering and anguish to the professional, the multi-professional team still identifies with and likes the area, which is important for the children and their families to feel safe, respected and well cared for by the professionals.

    Este artigo apresenta uma releitura de parte da obra de Elizabeth Ross, uma das autoras mais citadas sobre a questão da terminalidade da vida, do luto e do morrer. Sua obra tem sido de grande contribuição tanto para os profissionais de saúde como para pais, mães, filhos, parentes, leigos e religiosos que vivenciam o luto. Também tem sido alvo de controvérsias relacionadas a questões éticas e quanto a seu rigor científico. Os livros aqui comentados são: On death and dying (Sobre A morte e o morrer, de 1969); Questions and answers on death and dying (Perguntas e respostas sobre a morte e o morrer, de 1971); Living with death and dying(Vivendo com a morte e os moribundos, de 1981); On children and death (Sobre as crianças e a morte, de 1983); On life after death (Sobre a vida depois da morte, de 1991) e Life lessons (Lições de vida, de 2000).

    This article presents a reappraisal of part of the works of Elizabeth Kubler-Ross, one of the most quoted authors addressing the end of life process, mourning and dying. Her work has contributed to a clearer understanding of these issues by health professionals, families, religious and lay people who handle and/or experience mourning. She has also been the subject of controversy related to ethical issues and the scientific rigor of her work. The books analyzed in this article are: On death and dying (1969); Questions and answers on death and dying (1971); Living with death and dying (1981); On children and death(1983); On life after death (1991) and Life lessons (2000).
  • Eutanásia, distanásia e ortotanásia: revisão integrativa da literatura Revisão

    Atualmente, existe preocupação notória dos pesquisadores em debater questões geradoras de conflitos éticos, no âmbito assistencial, direcionada ao ser humano em fase de terminalidade, em particular, acerca da eutanásia, distanásia e ortotanásia. Este estudo objetivou caracterizar a produção científica, no âmbito nacional, acerca da eutanásia, distanásia e ortotanásia. Trata-se de uma revisão integrativa da literatura. O universo do estudo foi constituído por 41 publicações relacionadas ao tema investigado, mediante a realização de um levantamento online, na Biblioteca Virtual em Saúde, no Portal Capes e na Revista Bioética. Desses, 25 artigos compuseram a amostra, considerando-se critérios de inclusão e exclusão estabelecidos. A coleta de dados ocorreu em março de 2013, por meio de um instrumento com informações pertinentes ao objetivo proposto. Os descritores utilizados foram eutanásia, distanásia e ortotanásia. No que se refere ao enfoque das publicações, emergiram três temas: Tema I - Eutanásia; Tema II - Distanásia e Tema III - Ortotanásia. Os estudos analisados expressam a preocupação atual diante de dilemas éticos, no que concerne ao cuidar do ser humano na fase final de vida. Dessa forma, espera-se que esta pesquisa possa contribuir para fortalecer as leituras críticas a respeito da temática.

    There is currently widespread concern among researchers in debating questions that generate ethical conflicts within the scope of health care geared to the human being in the terminal phase, especially euthanasia, dysthanasia and orthothanasia. This study sought to characterize the scientific production at the national level on euthanasia, dysthanasia and orthothanasia. It involves an integrative review of the literature. The study universe consisted of 41 publications related to the theme in question by means of a survey conducted online in the Virtual Health Library in the Capes Portal and in the Bioethical Magazine. Of these, 25 articles comprised the sample taking into consideration the established inclusion and exclusion criteria. Data collection occurred in March 2013, by means of an instrument containing information pertinent to the proposed objective. The key words used were euthanasia, dysthanasia and orthothanasia. With respect to the focus of the publications, three themes emerged: Theme I - Euthanasia; Theme II - Dysthanasia and Theme III - Orthothanasia. The studies analyzed reflected the current concern in terms of ethical dilemmas concerning care of the human being in the end of life phase. Thus, it is hoped that this research can contribute to bolster the critical reading with respect to the theme.

    Com objetivo de discutir e compreender as experiências de oncopediatras diante da tarefa de comunicar notícias difíceis (ND) a pacientes e familiares procedeu-se uma revisão bibliográfica a partir das bases de dados da Biblioteca Virtual em Saúde (BVS) que contém artigos divulgados por PubMed, Lilacs, Scielo, Scopus e SciVerse, buscando a produção sobre o tema. Os artigos disponíveis, avaliados pela análise temática, evidenciam as seguintes questões: carência de estudos focando a percepção dos profissionais sobre as notícias difíceis; a comunicação como estratégia pela qual, aspectos psicossociais, culturais, bioéticos, legais e emocionais são tratados; expectativa de habilidades médicas para as quais esses profissionais não foram treinados. Observou-se também que informações e treinamento dos profissionais sobre estratégias a cerca dos modos de lidar com a comunicação de ND são insuficientes para acompanhar o dinamismo das ocorrências e das relações pessoais na área de oncologia onde o paciente costuma estar ou sentir-se em constante ameaça de vida.

    In order to discuss and understand the experiences of oncologists faced with the task of communicating difficult news to children and their close family members, a bibliographical review was conducted in the Virtual Health Library databases containing the literature published by PubMed, Lilacs, Scielo, Scopus and SciVerse with input on the theme. The articles available evaluated by thematic analysis reveal the following aspects: the lack of studies focusing on the perception of professionals about imparting difficult news; communication as a strategy by which psychosocial, cultural, bioethical, legal and emotional aspects are handled; that more is expected from physicians than they were trained for. It was also seen that the information and training of professionals in strategies regarding how to handle the communication of difficult news needs to be ongoing in order to keep up with the dynamism of the occurrences and the physician-patient relations in the oncological field in which patients tend to be or feel under constant life-threatening conditions.
  • Atitude frente à morte em profissionais e estudantes de enfermagem: revisão da produção científica da última década Revisão

    Profissionais de enfermagem são os integrantes da equipe de saúde que mais mantêm contato direto e prolongado com pacientes que vivenciam a terminalidade. Este estudo de revisão integrativa da literatura teve por objetivo investigar a atitude frente à morte em profissionais e estudantes de enfermagem. Os dados foram coletados por meio de buscas nas bases Lilacs, MedLine, PsycINFO e CINAHL, utilizando os descritores "nurses" e "attitude to death", no período de 2000 a 2011. Das 1376 referências levantadas, 262 foram selecionadas para extração dos dados e 35 foram recuperadas na íntegra, constituindo o corpus da pesquisa. Os resultados evidenciaram predomínio de artigos publicados em periódicos brasileiros. Os estudos indicam que o assunto morte e morrer têm sido negligenciados pelas instituições de formação, o que gera sofrimento entre profissionais e estudantes quando enfrentam a questão na prática, além de condutas inapropriadas diante dos pacientes que vivenciam o processo de morte. Como conclusão ressalta-se a necessidade de futuras pesquisas, que possam fornecer esclarecimentos mais pormenorizados a respeito do tema e busquem estratégias para suprirem a falta de preparo e respaldo dos profissionais de enfermagem ao vivenciarem a morte e o morrer.

    Nursing professionals are integral members of the healthcare team and they maintain the most direct and prolonged contact with patients experiencing a terminal illness. This integrative review of the literature sought to investigate the attitude towards death among nursing professionals and students. Data were collected through searches in Lilacs, Medline, PsycINFO and CINAHL databases using the key words "nurses" and "attitude to death" in the period from 2000 to 2011. Of the 1376 articles identified, 262 were selected for data extraction and 35 were downloaded in full, constituting the corpus of research. The results showed a predominance of articles published in Brazilian journals. Studies indicate that the subject of death and dying has been neglected in training institutions, which causes hardship among professionals and nursing students when faced with the issue in practice, in addition to inappropriate conduct when dealing with patients who are experiencing the end of life process. In conclusion, the need for future research that may provide more detailed clarifications on the subject and seek strategies to address the lack of preparation and support of the nursing staff when they cope with death and dying was emphasized.

    O despreparo para lidar com a morte e com a ausência do ente querido pode implicar em reações orgânicas e psicológicas que, em decorrência da capacidade adaptativa do indivíduo ao período de luto, acaba por interferir na condição de alimentação e, consequentemente, no estado nutricional. Este artigo aborda os efeitos do luto recente no comportamento alimentar, seguindo-se da análise de alguns pontos de vista da entrevista dietética; da condição de nutrição e de alimentação pós-morte do ente querido; e das implicações desse processo na fome, na sede e na culinária familiar, com enfoque nas condutas nutricionais e nas decisões que os norteiam. Trata-se de um estudo de revisão de literatura na temática morte e luto, o qual objetiva contextualizar o tema em reflexões pautadas nessa vivência, enfatizando a interação da nutrição com a ciência da tanatologia, área ainda pouco explorada e carente de estudos. A identificação dessa influência e de suas implicações permite melhor planejamento das estratégias alimentares, contribuindo sobremaneira para ações de enfrentamento e de apoio ao luto.

    The lack of preparation for dealing with death and the absence of the loved one may lead to organic and psychological reactions that, due to the adaptive capacity of the individual to the period of mourning, may result in interference in feeding habits and consequently on the person's nutritional status. This article addresses the effects of recent mourning on feeding behavior, followed by the analysis of the dietary interview from various standpoints. This includes the postmortem nutrition and feeding habits of the bereaved and the implications of this process on hunger, on thirst and on family cooking, with a focus on nutritional behavior and on the decisions that surround it. This is a review of the literature on the theme of death and mourning, which seeks to contextualize this theme around reflections based on this experience. It emphasizes the interaction of nutrition with the science of thanatology, which is an area still not properly examined and lacking study. The identification of this influence and its implications enables better planning of food strategies, contributing greatly to actions for coping and support during mourning.
