    OBJETIVO: Describir las causas y severidad de la anemia y los factores nutricionales asociados con hemoglobina en niños anémicos <5 años. MATERIAL Y MÉTODOS: Estudiamos los niveles de hemoglobina y las concentraciones séricas de ferritina, receptores solubles de transferrina, proteína C reactiva (CRP), zinc, hierro, cobre, magnesio, vitamina B12 y folato en 981 niños. Se utilizaron modelos de regresión ordinal y regresiones lineales múltiples para evaluar el riesgo de severidad de anemia y la variabilidad en hemoglobina. RESULTADOS: La prevalencia de anemia fue de 20.6%; el 14 y 6.38% tenían anemia leve y moderada. La anemia se asoció con deficiencia de hierro (DH) en 42.17%; la DH coexistió con deficiencia de folatos y vitamina B12 en 9%. Sólo 2% de la anemia se asoció con deficiencia de folatos o vitamina B12. CRP (coef: 0.17 g/dl) y el tercer tercil de cobre (coef: -0.85 g/dl) se asociaron con anemia sin explicar (p<0.05). CONCLUSIONES: DH es la principal causa de anemia en niños <5 años. Las concentraciones de folato y vitamina B12 se asociaron con anemia. La CRP se asoció con anemia sin explicar. Sin embargo, la deficiencia de vitamina A, que se asocia con anemia, no fue estudiada.

    OBJECTIVE: To describe the frequency and severity of anemia and the nutritional variables associated to hemoglobin levels (Hb) in children <5 years of age. MATERIALS AND METHODS: We studied 981 children measuring hemoglobin and serum concentrations of ferritin, soluble transferrin receptors (sTfR), C-reactive protein (CRP), zinc, iron, copper, magnesium, folate and vitamin B12. Ordinal logit or multiple regression models were constructed to assess the risk for anemia and the associations among nutritional variables. RESULTS: The overall prevalence of anemia was 20.6%, of which 14% were mild cases and 6.38% moderate. Anemia was associated with iron deficiency (ID) in 42.17% of the cases, whereas ID coexisted with either folate or vitamin B12 deficiency in 9%. Only 2% of cases of anemia were associated with either folate or vitamin B12 deficiencies. CRP (coef: 0.17 g/dl) and third tertile of s-copper (coef: -0.85 g/dl) were associated to unexplained anemia (p<0.05). CONCLUSIONS: ID is the main cause of anemia in children <5 y. Folate and vitamin B12 concentrations were associated with anemia. CRP was associated to unexplained anemia. However, vitamin A deficiency, which is associated with anemia, was not studied.

    OBJETIVO: Describir la magnitud de la deficiencia de folato y vitamina B12 en niños mexicanos. MATERIAL Y MÉTODOS: Se midieron las concentraciones séricas de folatos y vitamina B12 en una muestra probabilística de 2 099 niños. Se calcularon prevalencias ajustadas, medias de las concentraciones y las asociaciones relevantes mediante una serie de modelos de regresiones logísticas y lineales. RESULTADOS: La prevalencia global de deficiencia de folatos y vitamina B12 fueron 3.2% y 7.7%, respectivamente. La prevalencia más alta de folatos se encontró en el grupo de 2 años (7.9%) y de la vitamina B12 en los de 1 año de edad (9.1%). Ser beneficiario del programa de leche fortificada Liconsa estuvo asociado de manera protectora con el folato serico (p=0.001) y la ingestión diaria de leche con la de vitamina B12 (p=0.002). CONCLUSIONES: La deficiencia de ambas vitaminas, en los niños menores de dos años, es un problema de salud pública de magnitud moderada en México.

    OBJECTIVE: To describe the magnitude and distribution of folate and vitamin B12 deficiency in Mexican children. MATERIALS AND METHODS: Folate and vitamin B12 serum concentrations were measured in a probabilistic sample of 2 099 children. Adjusted prevalence, mean concentrations and relevant associations were calculated based on series of logistic and linear regression models. RESULTS: The overall prevalence of folate and vitamin B12 deficiency were 3.2% and 7.7%, respectively. The highest prevalence of folate was found in the 2-year-old (7.9%), and of vitamin B12 in the 1 year-old (9.1%) groups. Being a beneficiary of the fortified milk program Liconsa was protectively associated with serum folate (p=0.001) and daily Intake of milk with vitamin B12 (p=0.002) concentrations. CONCLUSIONS: We describe the magnitude of folate and vitamin B12 deficiencies in Mexican children. The deficiency of both vitamins in children under 2 years old is a moderate public health problem in Mexico.

    OBJETIVO: Describir el estado nutricio de micronutrimentos en niños de 1-11 años de edad de la Encuesta Nacional de Salud y Nutrición 2006 y su asociación con factores dietéticos y sociodemográficos. MATERIAL Y MÉTODOS: Se usaron muestras séricas (n=5060) para medir las concentraciones de ferritina, receptor de transferrina, zinc, cobre y magnesio. RESULTADOS: La prevalencias de deficiencias en niños de 1-4 y de 5 a 11 años fueron para ferritina, 26.0 y 13%; zinc, 28.1 y 25.8% respectivamente y cobre ≈30% en ambos grupos. Las concentraciones bajas de magnesio (CBM) fueron 12.0 y 28.4%, respectivamente. Ser beneficiario de Liconsa (RM=0.32; IC 95%: 0.17-0.61) y pertenecer al nivel socioeconómico alto (RM=0.63; IC, 95%: 0.41-0.97) fueron protectores para deficiencia de hierro. La edad (RM=1.26; IC, 95%: 1.19-1.32) y vivir en la región Centro (RM=0.59; IC, 95%: 0.36-0.97) fueron protectores para CBM. CONCLUSIONES: Las deficiencias de hierro y zinc son serios problemas de salud pública en niños mexicanos.

    OBJECTIVE: To describe the micronutrient nutritional status of a national sample of 1-11 year old Mexican children surveyed in 2006 in National Health and Nutrition Survey (ENSANUT 2006) and their association with dietary and sociodemographic factors. MATERIALS AND METHODS: Serum samples were used (n=5 060) to measure the concentrations of ferritin, transferrin receptor, zinc, copper and magnesium. RESULTS: Prevalence of deficiencies in 1-4 and 5-11y old children were for iron (using low ferritin) 26.0 and 13.0%; zinc, 28.1 and 25.8%, respectively; and copper, ≈30% in both age groups. Magnesium low serum concentrations (MLSC), were found in 12.0% and 28.4% of the children, respectively. Being beneficiary of Liconsa (OR=0.32; C.I.95%, 0.17-0.61) or belonging to higher socioeconomic status (OR=0.63; C.I.95%, 0.41-0.97) were protective against iron deficiency. Increasing age (OR=0.59; C.I.95%, 1.19-1.32) and living in the Central Region (OR=0.59; C.I.95%, 0.36-0.97) were protective against MLSC. CONCLUSIONS: Deficiencies of iron and zinc are serious public health problems in Mexican children.

    OBJETIVO: Describir la prevalencia de anemia y deficiencia de hierro, zinc, cobre y magnesio en adolescentes mexicanos en la encuesta probabilística ENSANUT 2006. MATERIAL Y MÉTODOS: La muestra incluyó 2447 adolescentes de 12 a 19 años de edad. Se tomó hemoglobina capilar y muestras de sangre venosa para medir las concentraciones séricas de ferritina, sTFR, CRP, zinc, hierro, cobre y magnesio. Se construyeron modelos de regresión logística para evaluar el riesgo de deficiencia de minerales. RESULTADOS: La prevalencia de anemia fue de 11.8% en mujeres y 4.6% en hombres. Las deficiencias de hierro fueron de 18.2 y 7.9% La deficiencia tisular de hierro fue 6.9%; la baja concentración de cobre fue de 14.4 y 12.25% la de zinc de 28.4 y 24.5%, la de magnesio fue 40 y 35.3% en mujeres y hombres, respectivamente. CONCLUSIONES: Existe una alta prevalencia de deficiencia de minerales en los adolescentes; las mujeres tuvieron mayor riesgo. Son necesarias intervenciones nutricionales para reducir o controlar estas deficiencias.

    OBJETIVE: To describe the frequency of anemia and iron, zinc, copper and magnesium deficiencies among Mexican adolescents in the probabilistic survey ENSANUT 2006. MATERIALS AND METHODS: The sample included 2447 adolescents aged 12 to 19 y. Capillary hemoglobin and venous blood samples were collected to measure the concentrations of ferritin, sTFR, CRP, zinc, iron, copper and magnesium. Logistic regression models were constructed to assess the risk for mineral deficiencies. RESULTS: The overall prevalence of anemia was 11.8 and 4.6%, body iron deficiency 18.2 and 7.9% for females and males, respectively. Overall prevalence of tissue iron deficiency was 6.9%, low serum copper were14.4 and 12.25%; zinc 28.4 and 24.5%, magnesium 40 and 35.3%; for females and males, respectively. CONCLUSIONS: There is a high prevalence of mineral deficiency in Mexican adolescents; females were more prone to have more mineral deficiencies. Nutritional interventions are necessaries in order to reduce and control them.

    OBJETIVO: Presentar un panorama del estado de micronutrimentos de niños mexicanos en 2006. MATERIAL Y MÉTODOS: Los datos sobre deficiencias de hierro, zinc, folatos, vitamina B12 y concentraciones bajas de cobre y magnesio se analizaron a partir de la Encuesta Nacional de Salud y Nutrición 2006. RESULTADOS: La deficiencia de hierro fue la más importante en niños (13-26%). La deficiencia de zinc fue alta en todos los grupos de edad (≈25%) y disminuyó entre 1999 y 2006 5.6 pp en niños. La deficiencia de folatos fue de 3.2% y la de vitamina B12 de 7.7%. Las concentraciones bajas de magnesio y cobre fueron altas (22.6 y 30.6%, respectivamente). CONCLUSIONES: La deficiencia de hierro pareció disminuir y la de zinc se redujo en niños mexicanos. La alta prevalencia de deficiencias de cobre y magnesio merecen más investigación para entender sus implicaciones en salud pública.

    OBJETIVE: To present an overview of micronutrient status of Mexican children in 2006. MATERIALS AND METHODS: Data on iron, zinc, folate and vitamin B12 deficiencies and low serum copper and magnesium were gathered and critically analyzed from the 2006 National Health and Nutrition Survey. RESULTS: Iron deficiency is still the main nutritional deficiency in children (13%-26%). Zinc deficiency was high in all age groups (≈25%) but reduced 5.6 PP in children <5 y from 1999 to 2006. Folate deficiency was 3.2% and vitamin B12 deficiency 7.7% in children. Low serum magnesium and copper were high (22.6% and 30.6%, respectively). CONCLUSIONS: The prevalence of iron deficiency seems to be lowering, and zinc deficiency has reduced in Mexican children. A high prevalence of copper and magnesium deficiencies warrants further research on their public health implications.

    OBJETIVO: Examinar el conocimiento y automedicación de antibióticos en adultos asegurados en México. MATERIAL Y MÉTODOS: Llevamos a cabo un estudio transversal mediante la administración de un cuestionario a 101 pacientes adultos que solicitaban atención médica por infección respiratoria aguda en una clínica de medicina familiar en México. La puntuación de conocimiento estuvo compuesta por respuestas correctas, incorrectas y "no sé", los factores asociados con conocimiento y automedicación de antibióticos fueron explorados mediante análisis bivariado. RESULTADOS: 47% de los participantes tomaron antibióticos previamente y 20% fueron automedicados. La puntuación de conocimiento fue muy variable. Muchos de los participantes creyeron que tratamientos comunes para resfriado y tos eran antibióticos, como ambroxol (45%), Desenfriol (45%) y paracetamol (44%). Los participantes con mayor edad (>40 años) obtuvieron mejores puntuaciones de conocimiento. DISCUSIÓN: Las percepciones erróneas sobre antibióticos y su automedicación son comunes en adultos que buscan atención médica en México.

    OBJECTIVE: To examine knowledge of and self-treatment with antibiotics among medically-insured adults in Mexico. MATERIALS AND METHODS: We conducted a cross-sectional, interviewer-administered survey among 101 adult patients seeking care for acute respiratory tract infections in a family medicine clinic in Mexico. Knowledge scores were calculated as a composite of correct, incorrect and don't know responses. Factors associated with antibiotic knowledge and antibiotic self-treatment were explored with bivariate analyses. RESULTS: 47% of participants were taking antibiotics prior to the visit, 20% were self-treating. Antibiotic knowledge was highly variable. Many participants believed common non-antibiotic treatments for colds and coughs were antibiotics, such as ambroxol (45%), Desenfriol (45%) and paracetamol (44%). Older participants (>40 years) had better knowledge scores. DISCUSSION: Self-treatment with and misperceptions about antibiotics are common among medically insured adults seeking medical attention in Mexico.

    OBJETIVO: Evaluar el uso y comprensión del etiquetado nutricional posterior (NIP, por sus siglas en inglés) de alimentos preempacados por consumidores mexicanos. MATERIAL Y MÉTODOS: Se aplicaron un cuestionario y una prueba de comprensión del NIP a consumidores adultos en supermercados de seis ciudades de las regiones Norte, Centro y Sur de México. RESULTADOS: Se entrevistaron 731 consumidores; 71.5% eran mujeres, la media de edad 33 ± 9.7 años (intervalo 18-60); 70% terminaron preparatoria o un nivel más alto. El 17% usa la etiqueta nutrimental para elegir sus alimentos; 49% no comprendía la NIP. Sólo 1.2% de los consumidores respondió correctamente las cinco preguntas de la prueba de comprensión del NIP. CONCLUSIONES: El uso y comprensión del NIP fue bajo, a pesar de la gran proporción que autorreportó leerlo y comprenderlo. La falta de conocimiento previo del lenguaje técnico dificulta el uso e interpretación de la información nutricional de NIP para las decisiones de compras de alimentos.

    OBJECTIVE: To assess the use and understanding of the Nutritional information Panel (NIP) of pre-packaged foods by Mexican consumers. MATERIALS AND METHODS: A questionnaire and an understanding test for NIP were applied to adult consumers in supermarkets of six cities in the Northern, Central, and Southern regions of Mexico. Data were analyzed by frequencies and Poisson regression models. RESULTS: Interviewed 731 consumers; 71.5% were women, mean age 33 ± 9.7 (range: 18-60), 70% completed high-school or a higher degree. In total, 17% of consumers use the NIP for making purchase decisions; 49% did not understand the NIP. Only 1.2% of consumers answered correctly the five questions of the NIP understanding test. CONCLUSIONS: The use and understanding of the NIP are low despite a high proportion self-reported reading and understanding. The lack of previous knowledge of the technical language prevents use and interpretation of NIP nutritional information for purchasing decisions.
  • Mutaciones asociadas con resistencia a rifampicina o isoniazida en aislamientos clínicos de M. tuberculosis de Sonora, México Artículo breve

    OBJETIVO: Realizar el análisis de regiones específicas de genes asociados con resistencia a isoniazida o rifampicina. MATERIAL Y MÉTODOS: Se estudiaron 22 cepas de M. tuberculosis, aisladas en Sonora, México. Se utilizaron iniciadores para regiones específicas de los genes rpoB, katG e inhA y la región ahpC-oxyR. Los productos de PCR se secuenciaron y analizaron. RESULTADOS: Se identificaron mutaciones en la región promotora del gen inhA, región ahpC-oxyR, codón 315 del gen katG y codones 451 ó 456 del gen rpoB. CONCLUSIONES: La identificación de mutaciones no descritas previamente obliga a continuar el análisis genotípico de cepas aisladas en Sonora.

    OBJECTIVE: To perform the analysis of specific regions of the major genes associated with resistance to isoniazid or rifampin. MATERIALS AND METHODS: Twenty two M. tuberculosis strains, isolated from human samples obtained in Sonora, Mexico. Specific primers for hotspots of the rpoB, katG, inhA genes and the ahpC-oxyR intergenic region were used. The purified PCR products were sequenced. RESULTS: Mutations in the promoter of inhA, the ahpC-oxyR region, and codon 315 of katG and in 451 or 456 codons of rpoB, were identified. CONCLUSIONS: Detection of mutations not previously reported requires further genotypic analysis of Mycobacterium tuberculosis isolates in Sonora.

    La obesidad en México es un problema de salud preocupante por el incremento en la prevalencia en adultos y niños, y se considera un factor de riesgo para el desarrollo de resistencia a la insulina, así como de otras alteraciones metabólicas. En esta patología se ha observado un incremento en la expresión de los receptores tipo Toll (TLRs) en el adipocito, receptores con participación crucial en la respuesta inmune innata. Se propone que los TLRs están implicados en la inflamación sistémica y en el desarrollo de la resistencia a la insulina. La activación de los TLRs es mediada por ácidos grasos y su expresión está regulada por leptina, adiponectina y PPAR. El conocimiento de la función de los TLRs, tanto en la inflamación como en la diferenciación del adipocito es importante en la búsqueda de nuevos blancos terapéuticos antiinflamatorios que coadyuven en el tratamiento de la obesidad.

    Obesity in Mexico is alarmingly increasing in prevalence in adults and children, and it is a risk factor for the development of insulin resistance, as well as, of other metabolic alterations. The discovery of the expression of the Toll-like receptors (TLRs) in adipocytes, suggests an important role in innate immunity. In different models of obesity, there has been observed an increase of TLRs expression in the fat tissue, therefore TLRs could be involved in systemic inflammation in this disease, and in the development of insulin resistance. TLR activation is mediated by fatty acids and their expression is regulated by leptin, adiponectin and PPARs. Knowledge of the role of TLRs in inflammation and adipocyte differentiation and their regulation, then it is important to try to develop new therapeutic anti-inflammatory targets that contribute in the treatment of obesity.
